Survey Monkey.com
Go to the "survey monkey" website. At the website, follow the directions to create a survey. Sign up for the "basic" subscription (free-see below) and create a ten question survey directed to parents or members of an organization. You don't have to actually collect information. Send me the URL (address) so I can see your survey. Try to options that allow you to create different kinds of questions, e.g., yes/no, multiple choice, short answer, etc.
