Complete the heading information.
Copy the Learner Outcomes from your syllabus and insert into the first column of the form.
Copy the Performance Indicators from your syllabus and insert into the second column of the form.
Copy the Assessment Methods from your syllabus and insert into the third column.
Enter the product type and assign it a number in column 4 of the Student Work Sheet.
Label the student's exemplary work sample with the same number.
Please mail/deliver the form and student work samples to the following persons:

- Undergraduate Education/M.I.T.- Administrative Assistant (Toppenish Campus)
